Zirconia vs. Metal Crowns: Why We Only Use Metal-Free Restorations

At Uttam Microdentistry, Dombivli, we do not use metal-based crowns - and for very good reason. Here is why Zirconia and E-max are superior in every way.

A Brief History of Dental Crowns

For most of the 20th century, dental crowns were made of gold or other metal alloys. Gold is actually an excellent dental material - it is durable, biocompatible, and easy to work with. However, it is obviously not tooth-coloured. The solution that emerged was the porcelain-fused-to-metal (PFM) crown: a metal substructure covered with a layer of tooth-coloured porcelain. This gave better aesthetics, but came with significant drawbacks.

Today, at Uttam Microdentistry in Dombivli, we have moved entirely to Zirconia and E-max (lithium disilicate) crowns - and here is exactly why.

The Problem with Metal-Containing Crowns

1. The grey gum line: As gum tissue recedes slightly with age (a normal process), the metal margin at the base of a PFM crown becomes visible as a dark grey line at the gum line. This is aesthetically very obvious, particularly on front teeth, and cannot be fixed without replacing the crown entirely.

2. Metal ions and tissue reactions: Some patients have mild sensitivity or allergy to the metal alloys used in PFM crowns. Metal ions can leach into surrounding tissue over time, causing a grey discolouration of the adjacent gum.

3. Opacity: Metal blocks light. Natural teeth transmit and reflect light in a complex way that gives them their characteristic glow. A PFM crown's metal substructure blocks this, making the crown look opaque and lifeless compared to natural teeth.

4. Porcelain chipping: The porcelain layer on PFM crowns can chip or fracture, exposing the metal underneath and requiring crown replacement.

Why Zirconia Crowns Are Superior

Zirconia (zirconium dioxide) is a high-strength ceramic material. Modern dental-grade Zirconia has a flexural strength of over 1000 MPa - stronger than natural tooth enamel and far stronger than porcelain. Key advantages:

  • Strength: Can be used anywhere in the mouth, including back molars that experience very high biting forces.
  • Aesthetics: No metal means no grey gum line. Modern monolithic Zirconia has excellent translucency, mimicking natural teeth.
  • Biocompatibility: Zirconia is completely biologically inert. It does not react with surrounding tissues and does not leach anything into the body.
  • Precision: CAD/CAM milled Zirconia crowns are fabricated by computer to fit with extreme precision, giving better marginal adaptation than hand-layered PFM crowns.

Where Do E-max Crowns Fit In?

E-max (lithium disilicate) crowns sacrifice some of Zirconia's brute strength in exchange for superior aesthetics. Their unique crystalline structure gives them optical properties very close to natural enamel - they transmit, scatter, and reflect light almost identically. For front teeth, where aesthetics are the priority and biting forces are lower, E-max is often our first choice. For back teeth and implant crowns, Zirconia is more appropriate.

What About Cost?

Zirconia and E-max crowns cost more to fabricate than PFM crowns. However, they also last longer, look better, and avoid the complications that lead to early replacement. When you factor in the likely need to replace a metal-containing crown due to gum line issues, chipping, or tissue reactions, the total cost of ownership is actually very similar - and the experience is far superior.
